We are centered on lifelong relationships. We have a 98% client-satisfaction rating and 75% return-rate because we pride ourselves in listening attentively to our clients’ needs. We provide a specialized and unique case-management program which has a 100% patient follow-up and aftercare program.
We provide holistic and personalized patient-centered care. Our hours are fitted to our clients needs. We offer 100% in-depth physical exams, not one-size-fits-all care. We have a network of resources and partnerships throughout the United States committed to our clients.
We are a community cooperative. All money raised here, stays here. We are 100% community owned and community operated. At least 75% of employees are 9th Ward residents. We have a tax-incentive partnership with businesses to provide quality health care to employees without health insurance.
We measure our success through results in patient outcomes. We measure success through clinical results of disease management, as well as, cost of quality healthcare delivery. No other factor could measure our work.
We listen, authenticate, and empower our residents to lead. We reduce barriers to quality care by preparing and utilizing our community members to become resource people and problem-solvers. Our clients are partners in their healthcare plan.
We let our community members speak of the work of our clinic. We are committed to maximizing community exchange by providing a meeting space to share innovative ideas, peer-to-peer education, workshops, and outreach.
